What telehealth is

Telehealth is the delivery of health care services using electronic communications, such as secure messaging, questionnaires, video or phone, when you and your clinician are not in the same place.

Benefits and risks

Telehealth can improve access and convenience. Risks include incomplete information, technical failures, and the absence of a physical examination, which may limit what a clinician can determine.

What you understand

Your clinician decides whether telehealth is appropriate and may refer you for in-person care. You may withdraw consent at any time. Your information is protected under applicable privacy law.

Your consent

By continuing, you consent to receive care through telehealth from Locumtele, LLC and its licensed clinicians.
